Question
Dear Dr.,
really need your help here - have experienced a sudden loss of vision in my right eye and still have no diagnosis.
A bit of background:
25 years of age
Highly myopic on both eyes, RE is -12, LE is -10, since childhood
Use glasses, disposable soft contact lenses once in a while when i run etc.
Take a lot of stress
Slightly overweight
The problem:
I dont know when it started but i noticed it about 2 weeks ago - that the vision in my right eye has decreased (from 6/6 with spects to 6/18). Had last checked it 4 months ago when it was 6/6 with spects - so it definitely happened sometime in these 4 months.
I noticed it while looking at a spreadsheet - i just coudnt read properly. Left eye is ok. The only other abnormality i saw was that if i blink too fast (on the right eye) or try to squint at a bright surface, i see a series of spots (each like the grainy screen that you see on a TV with no channel) - 3 circles in the central line and many small small dots forming a concentric circle around the central like. All this in the middle of the eye, periphery is unaffected.
The tests:
Did everything from basic examination to angio to OCT to a pattern response (EMG) to perimetry. Everything is normal.
Still clueless on the diagnostic? Am a bit worried, one of the doctors said its the onset of macular degeneration but the other one rebuted it saying the layered OCT is not showing it.
Any ideas what it could be? Really worried here.

Answer
Hi Ankur,
This sounds like Central Serous Retinopathy (CSR), however, needs to be diagnosed by a good retinal expert.
Most cases it is self limiting and it disappears in few weeks.
Commonly seen in young males 25 to 40 years of age and stress is a contributing factor. The stress raises the levels of cortisol which leaves a thin fluid like substance in the retina in the centre- macula.
However, this is just one of the differential diagnoses, only an ophthalmologist can confirm this.
If in Mumbai, we can examine you for the same.
