An unusual question

Question
What subtle impossibilities might an optometrist notice (without equipment) in an eye that an untrained observer would miss? I'm writing a book and would like some small abnormalities for one of my characters' eyes. I realize this has no medical basis and thank you in advance for your time.

A good optometrist will be able to make out that the person has undergone a cataract surgery or a lens replacement surgery where the 'shiny glow' of the lens is apprecaited by them, but a untrained observer would not be able to make out this subtle sign.
